Advance Healthcare Planning in the Cleveland LGBTQ+ Community

Did you know…
Advance Directives let others know what types of medical care you want and who you want
involved in circumstances if you are not able to participate in your own medical decisions? Many
in the LGBTQ+ community lack these important documents.
Please join this important conversation to:

• Reflect on your medical wishes.
• Engage in conversation to help others understand those wishes.
• Complete Advance Directives to share with your loved ones and medical providers.

~30 minute presentation to begin at 4:30pm, followed by Q&A
-Space available for private consultation and assistance with completing Advance
Directives on site.
-Community members are welcome to stop by whenever is most convenient for them!
They do not need to be present for the full two hours.
-Fun raffle prizes will be available to participants!
